Go/no-go evidence card
- Report four families: progress, coverage, quality, risk. Pair every progress number with coverage or quality.
- Define each metric in one line; define RAG against exit criteria and always show the reason.
- Daily: blockers and actions. Weekly: per value stream plus decisions needed. Steering: answer, reasons, asks.
- Plot arrivals and closures by severity; read them against execution volume.
- Forecast: open above threshold ÷ net closure rate vs days left; state assumptions and reopen rate.
- Pack: scope, traceability, results, open defects, residual risk, exit criteria, sign-offs, migration and cutover inputs.
- Open critical/high defects in business language: process, workaround, owner, fix date.
- Test the pack: can an outsider trace three risks and two requirements?
- Recommend go, go with conditions or no-go; conditions specific, owned, dated, verifiable.
- Record the decision, conditions and accepted risks with the pack.
