Governance and RAID card

• Every forum has a charter: purpose, members, decision rights, inputs, outputs, cadence.
• One decider per decision type; thresholds trigger escalation automatically.
• Test lead signs evidence; process owners sign acceptance; sponsor signs residual risk.
• Gate criteria = condition + evidence + owner, agreed at the start of the phase.
• Gate outcomes: pass, pass with specific owned and dated conditions, or fail.
• RAID: risks with probability, impact, response and trigger; assumptions with validation dates.
• One named owner per RAID item; weekly review; top items with trends to steering.
• Define green, amber and red by what they ask of decision-makers.
• Pair activity metrics with outcome metrics from Cloud ALM; watch leading indicators.
